EULAR 2026: Synovial Biopsy-Driven Treatment and Weight Loss Improve Psoriatic Arthritis Outcomes

Two studies presented at the 2026 EULAR Congress highlight key advances in PsA management.

Synovial Biopsy-Driven Treatment

A real-world proof-of-concept study compared a biopsy-driven therapeutic strategy to standard care in 35 PsA patients. Synovial samples were classified into three pathotypes: myeloid, lympho-myeloid, and pauci-immune.

15 patients with a myeloid CD117-positive pathotype received biologics directed against IL-17, IL-23, or TNF; 20 controls received standard treatment.

Low disease activity or remission was achieved in 68% of the biopsy-driven group vs. 45% of controls. Among myeloid CD117-positive patients, IL-17 and IL-23 inhibitors led to greater DAPSA reduction at 6 months than TNF inhibitors.

Simone Parisi presented the findings, noting that a synovial biopsy-driven strategy improved clinical outcomes.

Dietary Interventions for Weight Loss

The DIPSA trial randomized 92 overweight/obese PsA patients (mean BMI 33) to Mediterranean diet, low-calorie diet, or standard-of-care dietary advice.

All groups achieved modest weight loss by week 12; no significant between-group differences in DAPSA reduction at weeks 12 or 24.

Greater weight loss was associated with better clinical outcomes regardless of diet type.

Lihi Eder presented the results, suggesting weight loss is an effective adjunct strategy.
