Clarkson revealed during a conversation with farmworkers Kaleb Cooper and Charlie Ireland on the season finale of Clarkson's Farm that he has cancer. He stated he had known of the diagnosis since May. Multiple sources reported Clarkson described the form of cancer as "aggressive."

Following the broadcast, Clarkson posted an update on Instagram. In the video, he stated he is "not dead" and is "perfectly fine." He explained that doctors discovered the cancer early as a result of a blood test. He urged viewers to seek similar testing.

Clarkson has undergone treatment for prostate cancer using High-Intensity Focused Ultrasound (HIFU), a minimally invasive procedure that uses a probe inserted into the rectum to destroy cancer cells.
According to Clarkson, a follow-up PSA test conducted two months ago indicated his cancer is in remission. He noted that up to 40% of prostate cancer patients experience a recurrence and stated he will continue to monitor his condition.
In an interview with The Times, Clarkson reported that complications arose during his treatment because he resumed taking tablets for vascular and cardiac conditions without consulting his doctors.
